Health Data Works was founded in 2021 with the mission of providing a uniform platform of comprehensive dialysis access management to serve patients and caregivers in a value based health care system.

Our software is designed to provide complete tracking and analysis of all touchpoints involved with dialysis patient's care and enhance delivered care and outcomes consistent with value-based care model.

Our software is comprised of a photo record of patient's access with superimposed symbolic representation of prior access points, time since access, critical features of the access such as stent, stenosis or aneurysm location and graft function.
